Application Requirements

Online Application

Complete the online application form.

Recommendation

Students are required to submit 1 letter of recommendation. Letters from an NP, PA, MD, supervisor, or faculty member are strongly recommended.

RN License Verification

Include your RN license number in your application for verification. Students are expected to have a current, unencumbered MN license to enroll. Students with a license from another state are required to obtain a MN license prior to starting in the program.

Clinical/Practice Hours (only masters-prepared applicants)

Documentation of practice hours for Masters-Prepared applicants will be requested upon acceptance to the program. A maximum of 600 practice hours may be transferred from a nationally accredited masters in nursing program. If the university/college you attended does not record the practice hours on your transcript, a letter from your college or university department can be submitted upon acceptance to the program.

Transcripts

Applicants must submit an official transcript(s) from the regionally-accredited institution granting the Bachelor’s degree and from the most recently attended institution, if not the same. Military veterans are required to submit all official transcripts from previously attended institutions to qualify for military benefits.

In the case where prerequisite requirements exist or transfer credits or waivers are sought, official transcripts are required from institutions at which the prerequisites or equivalents were completed. The Program Director may ask for additional transcripts if determined to be necessary.

Submit transcripts directly to the Graduate Admissions Office by electronic transfer or by mail.
